How to Download ESIC Form 7B Online
Only your employer can download your ESIC form 7B medical acceptance card in their employer ESIC portal.
Step 1:- To download ESIC form 7B login to employer portal with their establishment code and password at www.esic.in.
Step 2:- After login to this portal, now click on print counter foil under employee.
Step 3:- Now enter ESIC IP number of the insured person and click on the print counter file.
Step 4:- Now employee print counterfoil will appear on the screen, at the top left side of the screen a link to download medical acceptance card pdf file or ESIC form 7B pdf download option of that particular employee will be available.

Step 5:- Now click on that link and download ESIC form 7B or ESIC medical acceptance card. Which contains insured person name, insured person’s father name, factory name, residential address of the insured person and IP number of the insured person.

How to Download ESI Medical Acceptance Card Online
ESI medical acceptance card can be downloaded from online ESI employer portal. Your employer need to login using employer login id and password. If you are working in a small organization where HR team will not be strong and may not know all this procedures. In that case you can help them to get your med 7b card.
Step 1: Login to ESIC Employer Portal.
Step 2: Under employee, you can find print counter foil link. click it.
Step 3: Now, you need to enter ESIC IP number of the insured person and click on the print counter foil.
Step 4: Now employee print counterfoil screen will be displayed. On this page you can find a link titled ‘Medical Acceptance Card (Click here to view medical acceptance card)’. Once you click that link, ESIC Medical Acceptance Card Form 7b will be downloaded.
Along with this form, insured person should also submit following documents as well.
- ESIC Card’s Xerox attested by employer.
- E-Pehchan Card or Temporary Identity Certificate with Aadhar card number attested by employer.
- Entitlement to Benefit Certificate
